Administrative Approval - employed to override a pre-/co-requisite requirement or maximum registered hours for a term
Arranged Basis - Independent Study Registration - employed to communicate student enrollment on an arranged-basis or for an independent study course
Incomplete Grade Request - employed to assign an Incomplete grade
Registration-Information Change - employed to communicate registration activity undertaken by an advisor on a student's behalf and/or changes in basic student record data
Request for Grade Change - employed to communicate a faculty member's desire to revise a previously recorded final grade
Special Case Forms
Huddle Form - employed to document the involved student, reason for the Huddle, and outcomes.
FERPA Waiver - employed to authorize third-party access to FERPA protected student record information; not available online. Please direct the student to contact the Office of Records and Registration for assistance with filing this form
