Course Registration-Related Forms

Course Request - employed to request a seat in a 'full' course (either at capacity or zero seats available) on or after the first day of a term once CAMS Student Portal registration functions are suspended

Drop/Withdrawal - employed to request removal of one or more courses from the student's registrations; Drop or withdrawn statuses will be determined based on the submission date in relation to the current Academic Calendar deadlines

Registration (Course Add) - employed to request that one or more courses be added to a student's schedule on or after the first day of a term once CAMS Student Portal registration functions are suspended and when seats remain available in the requested courses

Administrative Forms

Change of Name - employed to update the student's record on any occasion when the student's legal name has changed

College Withdrawal - employed when the student wishes to communicate an intent to fully withdraw from, and cease being a student of, the College

Enrollment Verification - employed when the student wishes to have aspects of their enrollment records with the College verified via a formal letter (or form provided to the Office of Records and Registration) with a third-party (e.g. potential employer, state agency, etc.)

Final Grade Appeal - employed when the student wishes to appeal a final course grade

Gerontology Certificate Declaration - employed when the student wishes to add the requirements pursuant to the Gerontology Certificate to their adopted curricular requirements

Leave of Absence - employed to request a temporary Leave of Absence from the College for a period not exceeding 180 days in length

Letter of Recommendation Request - employed when the student wishes to request that a faculty member or other College representative write a Letter of Recommendation on the student's behalf

Minor Declaration or Change - employed when the student wishes to add or remove a Minor designation from their adopted curricular requirements

Program Change/Re-declaration of Major (Non-Nursing Programs) - employ when the student seeks to change program of study into a Non-Nursing program

Program Completion/Graduation Application - employed during the student's second to last or anticipated final term of attendance to communicate the student's intent to graduate from their adopted program of study

Request to Prevent Disclosure of Directory Information - employed when the student wishes to restrict the disclosure of Directory Information and attend the College anonymously in relation to third-parties

Nursing Program-Specific Forms

Change of Degree Request Form - employed by students wishing to request a change of program of study that also would result in a change of degree level (i.e. moving from a baccalaureate program to a graduate program)

Medical Terminology Challenge Exam Request - employed by students in the LPN to BSN program who wish to attempt the Medical Terminology Challenge Exam in satisfaction of the HS160 "Medical Terminology" course requirement

Nursing Readmission Application - employed by students who have been academically dismissed or who have withdrawn from the College and wish to be readmitted to the Nursing programs of study

Program Change - Nursing, Prelicensure (traditional) - employed by students who are not currently declared for a Nursing program of study and wish to seek admission to the traditional BSN Prelicensure program

Program Change - Nursing, Accelerated Prelicensure - employed by students who are currently declared for the traditional BSN Prelicensure program and wish to seek admission to the Accelerated Prelicensure cohort
